I do not think it is the game disc itself, but something on my xbox somewhere that is causing it to not want to load or save to the hard drive.
I tried playing without my hard drive, and the disc read fine and played a new game without an issue and I was able to create a character. This same error comes up when I select "continue" or "load." When I start a new game it will play through fine up until the character creation and then crash with the error.

My xbox started up, the game loaded right away to the title screen, but when I went to "Continue" it would give the same error saying the disc could not be read and to try cleaning it.
